The following instructions apply to Declaration of Interest for Cochrane roles. See Declaration of Interest forms: instructions for authors if you are a Cochrane author and need to complete a Declaration of Interest for a Cochrane Review.

People holding certain roles within Cochrane are required to submit a Declaration of Interest. Your Declaration should be updated in January of each year, or whenever there is a change in your circumstances. This demonstrates that your Group is complying with Cochrane's Conflict of Interest Policy. This is divided into two parts, based on whether your role involves the production of Cochrane Library content.

Please note that individuals who hold multiple roles must submit a separate declaration for each role.

Maintaining accurate and up-to-date Declarations for staff in key roles is critical to Cochrane’s reputation. Please check your Cochrane Account now and ensure the information is up to date

Members of the Governing Board declare all potential conflicts of interest for the previous ten years. For other roles the relevant period is three years.

Completed forms are publicly available on Cochrane's organisational chart of Declarations of Interest. Submitted declarations can take up to 24 hours to update on the website.

Submit a Declaration of Interest form

  1. Log in to the Roles tab on your Cochrane Account.
  2. All your active Cochrane group roles will be listed alphabetically by role.
  3. Click 'Submit Conflict of Interest form' for the relevant role.
  4. The form will open in a new browser window.
  5. Complete the form and click Save.
  6. Click OK to confirm saving.
  7. Return to the Roles tab on your Cochrane Account and refresh the page to view the submitted form and the updated date of submission.
  8. If the same form is valid for more than one role (e.g. Editor, Managing Editor), you only need to complete the form once. It will automatically update for all relevant roles.

  3. Click 'View Conflict of Interest form' for the relevant role.
  4. The form will open in a new browser window.
  5. Update the form and click Save.
  6. Click OK to confirm saving.
  7. Return to the Roles tab on your Cochrane Account and refresh the page to view the submitted form and the updated date of submission.
  8. If the same form is valid for more than one role (e.g. Editor, Managing Editor), you only need to update the form once. It will automatically update for all relevant roles.
